Areas of ground will sometimes vanish when digging or adding dirt. These are the console errors that showed up when it happened.

So far there has been no acknowledgement from the devs about this SERIOUS issue. The last post in the last thread I linked to has a solution that worked for me and the other poster, in summary:

To fix it you need to find the tiny stray polygon and place a wood frame box over it... unfortunately this is easier said than done as the tiny polygon can be in the air facing the other direction at the original soil level. It will be close to one of the tips of the bad large easily visible polygon that appears.

We have used this fix several times now.

I have an issue with it as well, most of the time i can reproduce. The errors i get followed by pics:

Failed setting triangles. Some indices are referencing out of bounds vertices. IndexCount: 9 VertexCount:280 (thats listed twice followed by:)
Failed getting triangles. Submesh 1 has no indices. Mesh asset path ""Mesh name ""

All I was doing was building a 1x1 stone wall from ground up against a dirt mountain wall already there.

Last shows the wall. Right before this happened after I place a stone down - a longggggggg triangular piece of something went from the stone i place 10 high all the way to the ground - failed to get screenshot.

After logging back in - after falling through the world (and repeated error messages about player being stuck) I was able to get this screenshot:

after battling - quitting, re-enter game, place a couple frames, fall thru world, quit....rinse repeat I was able to put wood frames up against the entire affected wall (attempting to flatten the face of wall) it "seems" to have fixed the issue. will report if it repops.

I recently saw it happen in a Let's Play (SP no mods) - it weirdly happened directly after a shotgun was fired...but obviously the lets player didn't show the console...

I got this quite often with 12.3 or such, less so with 12.4 But the longer the seed is up, the more world issues we get.

I am experiencing this issue on my Server currently. I have tried all I can to remove, or replace the block doing this, but its hard to determine what block is the culprit as every move I make the giant dirt triangle vanishes. Then I move again, BOOM, im stuck inside a giant dirt pyramid.

Funny that Ultix is the one who posted this. I hope he isn't experiencing this on my server currently.
